Contract will be extended to end of March 2013,
Although times for these roles will initially be 9-5 Monday to Friday, the service is under review and
there is the potential to be working outside these hours/days in the future
The Team(s) provide assessment and care management services, advice, guidance and information for adults including
support to and assessments of Carers. There are 5 teams based across the County – 4 Adult care teams in each of the former
districts and one county-wide team providing services for people with learning disabilities. Following contact the service
determines whether a full Community Care Assessment is provided and following assessment the team, with the customer
will determine the support needed, whether the services and support are eligible for Council funding and how those services
and support are going to be delivered to enable people to live as independently as possible whilst achieving their desired
outcomes. The Team(s) have a primary responsibility for safeguarding vulnerable adults and will following an alert, determine
appropriate action in conjunction with other agencies as appropriate.
This job is also one within a Hospital Adult Care Team, where the roles differ slightly in that the team supports effective
discharge from hospital settings, ongoing support services to maintain independence and referral on to a Locality Team where
appropriate.
