Community Learning Disabilities Team –1x Transition Social Worker

We are looking to recruit a Social Worker to a temporary post (Temporary for 3 months starting from Monday 2nd December 2019) to focus on children with complex disabilities within educational and specialist care facilities who are likely to transition to Adult Social Care Learning Disability Services within the next 12 – 36 months. 
The applicant will have to engage with a range of agencies to illicit this information and present their findings in an agreed and consistent format to support resource planning and service development.


Community Learning Disabilities Team -  2 x Social Worker 

We are looking to recruit 2 Social Workers to temporary posts (Temporary for 3 months from the start date Monday 2nd December 2019) that focus on care planning and review of individuals presently in receipt of services to meet their outcomes under the SSWBA (14). Applicants will focus on individuals who are supported by the Adult Community Learning Disability Team to develop individualised packages of care within a framework of positive consultation and joint planning. Care plans will aim at enabling people with a learning disability to develop their maximum potential as individuals and to ensure their personal outcomes are identified and supported to allow progression to greater independence.

Experience of working with people with learning disabilities is desirable.
